Getting Started
The first in step in getting started is to reach out to our intake coordinator by filling out this FORM. It is important you fill out all form fields.
Our office will contact you
for a brief phone consultation to see if we are a good fit, review insurance & cost and to set up an appointment.
Please be patient with us as we are doing our best to keep up with all emails & calls.
Rates & Insurances
During your complimentary consultation, our intake coordinator will review your insurance details and explain any out-of-pocket cost.
The rate for the initial intake session is $250 (50 minute visit & treatment plan development);
Subsequent session rates are $200 for a 45 minute session & $250 for 60 minute sessions.
As of Jan. 1, 2022 you have the right to a Good Faith Estimate. To learn more about the GFE

Cancellation Policy
We prioritize our clients by holding space for each person we see. When clients cancel an appointment, we will offer that space to someone who may need an appt that week. Without proper cancellation notice, other clients miss out on an opportunity to be seen.
For this reason we have a 24 hour cancellation policy.
Late cancellations or failed appointments will be subject to a $100 late cancel charge. This charge is not covered by insurance.
